Application form (for leads)
The application form is automatically sent to new leads when they sign up through your share link.
How to edit the application form (Pro only):
Go to your Leads page.
Click Preview to open the form.
Make changes and save.
💡 Tip: Use your application link on social media, your website, or landing pages to capture new leads directly into Superset.
Onboarding form (in client profile)
The onboarding form helps you collect key info from new clients. You can also choose which questions to include when assigning it.
Steps to assign:
Open a client profile.
Go to the Forms tab.
Click Add onboarding form.
Select the questions you want to include.
Click Save.
Check-in form (in client profile)
The check-in form lets you track progress with recurring or one-time questions.
Steps to assign:
Open a client profile.
Go to the Check-ins tab.
Click Create check-in.
Choose Recurring or One-time, set the day and frequency.
Select the questions from your form library (e.g. weight, stress, progress pics).
Save to assign.
⚠️ Note: Assigning forms is available on all plans. To customize the questions beyond the default options, you’ll need a Pro plan.
