How to add a PAR-Q form or liability waiver to a client profile

Attach PAR-Q forms and liability waivers in Superset by adding them to a client invite email or linking them in the client’s Content section

You can either send a PDF as an invite attachment (during Add client) or add a link inside the client profile under Content.

Follow these steps

  1. Open Clients.

  2. Do one of the following:

    1. Attach to the client’s invite email

      • Click Add client and enter their details.

      • On the final step (before sending the invite), click Add attachment.

      • Upload the PDF (PAR-Q, liability waiver, or any other PDF).

    2. Link it in the client profile

      • Open the client → go to Content.

      • Click Add content.

      • Paste the form link (Google Form, Google Doc, etc.).

      • Name it (example: “PAR-Q Form” or “Liability Waiver”) → click Save content.

Notes & best practices

  • Invite attachments are sent together with the invite email.

  • Use Content when you want clients to access a link anytime from their profile.

  • You can link any format that has a shareable URL (Google Form, Doc, etc.).

Can I send a PAR-Q or waiver as part of the invite email?

Yes. During Add client, click Add attachment on the final step and upload the PDF.

Can I add a waiver link to an existing client?

Yes. Open the client → ContentAdd content → paste the link → Save.

Does the attachment need to be a PDF?

Invite attachments are uploaded from your computer and sent with the invite email (commonly as a PDF).

What if my form is a Google Form or Google Doc?

Add it as a link in Content using Add content.
