How to set up coach application forms

Note: Form questions cannot be edited at this time.

To create an application form, follow the steps below:

Log in to your coach dashboard.
Select the “Leads” button in the upper left-hand-corner.
Click “Edit form” to toggle which questions you’d like to include in your form.
Share your application form by clicking “Copy link” and pasting it in your IG bio, LinkTree, website, etc.
