How to edit forms

Customize Superset forms by editing default questions, adding new question types, and updating your intro screen text

Forms let you collect information from leads and clients using the same question builder. Go to Forms to edit any form type.

Follow these steps

  1. Open Superset → Forms.

  2. Select the form you want to update.

  3. Turn the Intro screen on or off.

  4. Edit the intro text your client will see (if enabled).

  5. Update questions:

    • Edit or remove Superset’s default questions.

    • Scroll to the bottom and click Add question to create new ones.

  6. Choose a question type:

    • Multi-line text

    • Single select

    • Weight

    • Height

    • Date

    • Number

    • Progress photos

  7. Click Save.

Notes & best practices

  • You can edit questions for all form types (application, onboarding, and check-ins).

  • Keep the intro screen short and tell the client what to expect (time to complete, what you’ll use it for).

  • Use single select for faster answers and easier tracking.

  • Use number/weight/height/date when you want structured data (not free text).

  • Use progress photos when you want consistent visual check-ins over time.
