Forms let you collect information from leads and clients using the same question builder. Go to Forms to edit any form type.
Follow these steps
Open Superset → Forms.
Select the form you want to update.
Turn the Intro screen on or off.
Edit the intro text your client will see (if enabled).
Update questions:
Edit or remove Superset’s default questions.
Scroll to the bottom and click Add question to create new ones.
Choose a question type:
Multi-line text
Single select
Weight
Height
Date
Number
Progress photos
Click Save.

Keep the intro screen short and tell the client what to expect (time to complete, what you’ll use it for).
Use single select for faster answers and easier tracking.
