How to assign forms

Assign application, onboarding, and check-in forms so leads and clients can submit info directly in Superset

You assign different forms in different areas: Leads for the application form, and the client profile for onboarding forms and check-ins.

Follow these steps

Assign an application form (for leads):

  1. Open Superset → Leads.

  2. Click Share link (top right).

  3. Copy and share your link; new leads will fill out your application form from this link.

Assign an onboarding form (for a new client):

  1. Open Superset → Clients.

  2. Select a client.

  3. Scroll to Forms.

  4. Click Add onboarding form.

  5. Save to assign — the client’s results will stay saved in that client’s Forms section.

Create and assign a check-in (recurring or one-time):

  1. Open Superset → Clients → select a client.

  2. Scroll to Check-ins.

  3. Click Create new.

  4. Choose Recurring or One-time.

  5. Choose the day your client can reply.

  6. Set the cadence (ex: Weekly or Biweekly / every 2 weeks).

  7. Add internal notes (for you).

  8. Select the questions your client will receive.

  9. Save to assign.

Notes & best practices

  • The application form is shared via your Leads → Share link.

  • Onboarding form results remain visible in the client profile under Forms.

  • Check-ins only appear to the client on the day they’re due.

  • All check-in answers stay accessible in the client profile under Check-ins.

  • Use recurring check-ins for weekly tracking and one-time check-ins for milestones or intake follow-ups.

Where do I share my application form?

From Leads → Share link (top right).

Will onboarding form answers stay saved?

Yes, onboarding results remain in the client profile under Forms.

When can clients see a check-in form?

Only on the day it’s due. After they submit it, you can review answers anytime under Check-ins.

What’s the difference between recurring and one-time check-ins?

Recurring repeats on your selected cadence (weekly or every 2 weeks). One-time is a single check-in sent on the scheduled day.
